How We Collect, Use, Protect & Disclose Your Personal and Health Information

This document applies exclusively to VIP Regen Med LLC d/b/a VIPrivate Care Longevity & Anti-Aging. Prime IV Hydration & Wellness, operated at the same address by Health & Longevity LLC, is a separate non-medical wellness business with its own privacy practices, financial policies, systems, and forms. Nothing in this document applies to Prime IV services, and no information collected under this document is shared with Prime IV except as you separately authorize in writing.

1. Who We Are and What This Covers

VIPrivate Care Longevity & Anti-Aging ("we," "us," "our," or the "Practice") is a physician-directed medical practice operated by VIP Regen Med LLC at 295 W Grand Ave, Unit C, Montvale, NJ 07645. This Policy applies to information collected through our website, in our office, by telephone, text message, and email, through our scheduling and intake systems, and through any other interaction you have with the Practice. It does not apply to third-party sites or platforms we do not control, even where we link to them.

This Policy is published on our website and provided to patients. It is not a form you sign. Your protected health information is separately governed by VIPC-ADM-002.

 

2. Information We Collect

 

2.1 Information You Provide

  • Identity and contact details: name, date of birth, mailing address, email address, and telephone numbers.

  • Scheduling and account information: appointment requests, preferences, and communications with our staff.

  • Health information: medical history, current medications, allergies, treatment goals, consultation and procedure notes, clinical photographs, and laboratory or diagnostic results.

  • Payment information: billing address and card or financial account details, processed by our payment processor. We do not store full card numbers on our own systems.

  • Communication preferences: the consents you give or withhold under VIPC-ADM-002, and any later changes to them.

 

2.2 Information Collected Automatically

When you visit our website, we and our service providers may collect your IP address, device and browser type, pages viewed, referring URLs, and interactions with the site, using cookies, pixels, and similar technologies for functionality, analytics, and marketing measurement. You can control cookies through your browser settings; disabling them may affect site functionality.

 

2.3 Information From Third Parties

We may receive information about you from referring providers, your authorized representatives, and our scheduling, charting, payment, and analytics vendors, consistent with applicable law.

 

3. How We Use Your Information

  • To provide, coordinate, and manage your care and the services you request.

  • To schedule, confirm, reschedule, and remind you of appointments.

  • To communicate with you about your treatment, findings, and follow-up care.

  • To process payment and billing, and to administer memberships and packages.

  • To operate, maintain, and improve our systems and internal processes.

  • To meet our legal, regulatory, licensing, and professional obligations, including those of the New Jersey Board of Medical Examiners and the U.S. Food and Drug Administration.

  • To detect, prevent, and address fraud, security incidents, and other unlawful activity.

  • To send you marketing and promotional communications — only where you have given the separate consent in VIPC-ADM-002.

 

Health Information Is Treated Separately

Your protected health information is used and disclosed for treatment, payment, and health care operations as permitted by HIPAA and described in VIPC-ADM-002. We do not use or disclose protected health information for any marketing purpose that requires your authorization without first obtaining that authorization, and we do not sell your protected health information.

 

4. How We Share Your Information

We do not sell your personal information. We share it only as follows:

  • Service providers: scheduling and intake platforms, charting and practice management systems, communications platforms, payment processors, IT and hosting providers, and analytics partners. Where a vendor handles protected health information, it does so under a written Business Associate Agreement.

  • Treatment partners: other licensed providers involved in your care, including our Medical Director and consulting clinicians.

  • Legal and regulatory: when required by law, subpoena, court order, or to cooperate with regulators, licensing boards, or law enforcement under conditions the law specifies.

  • Safety: to prevent or lessen a serious and imminent threat to your health or safety or that of others.

  • Business transfers: in connection with a merger, acquisition, financing, or sale of assets, subject to confidentiality protections and applicable law. You will be notified as required by law.

 

5. Data Security and Retention

We maintain administrative, technical, and physical safeguards designed to protect your information against unauthorized access, use, alteration, and disclosure, including access controls, encryption where appropriate, staff training, and vendor due diligence. No method of transmission or storage is completely secure, and we cannot guarantee absolute security. In the event of a breach of unsecured protected health information, we will notify you and the applicable authorities as required by HIPAA and New Jersey law.

We retain medical records and related information for the period required by New Jersey medical record retention rules and for as long as necessary to meet our legal, regulatory, and professional obligations, resolve disputes, and enforce our agreements. When information is no longer required, we dispose of it securely.

 

6. Your Choices

  • Update or correct your contact and account information at any time.

  • Opt out of marketing calls, texts, and emails. See VIPC-ADM-002.

  • Limit cookies and similar technologies through your browser settings.

  • Request information about the categories of personal information we hold about you.

 

We will not discriminate against you for exercising any privacy right.

 

7. Minors

Our website and services are intended for adults. We do not knowingly collect personal information from children under 13 through our website. Where the Practice provides any service to a minor, it does so only with the consent and involvement of a parent or legal guardian, and only where the service is permitted for that patient's age under the Medical Director's Age-and-Service Eligibility Matrix. If you believe we have inadvertently collected information from a child, contact our Privacy Officer so we can address it.

 

8. Third-Party Links and Platforms

Our website and communications may link to third-party sites and platforms, including scheduling, payment, and social media services. We are not responsible for their privacy practices. Review their policies before providing them with your information.

 

9. Changes to This Policy

We may update this Policy to reflect changes in our practices, technology, or legal requirements. The revision date appears in the header. Material changes will be communicated as required by law, and the current version will be available in our office and on our website.
